How can I get the form?

The Street Improvement Petition form is available online and is used to prioritize the City's proposed construction program. Property owners may also visit the Engineering Department in person to pick up the form or may call 715-839-4934 to have it mailed.

Where do I turn in the completed form? 

All completed forms should be returned to the City Clerk.

How can I be sure my street will be included in next year's Report of the City Engineer?

Forms returned by October 1st of this year will be included in the Report of the City Engineer for next year.

What are the form requirements?

Signatures 

Property owners on the street must sign the petition but signatures of all property owners are not required.

 

Addresses 

The property owner's address must be specified on the petition.

 

Type of Improvements Requested

The type of improvements requested (bituminous paving, curb & gutter, etc.) must be specified on the form. The proposed project may include items not included in the petition, however. (For example, replacement of the sewer may be included in that proposed project if the condition of the sewer main indicates it should be replaced even though the petition only indicated that a specific street be repaved.)
